Avoiding a Malthusian Catastrophe - 783 Words

Thomas Malthus once said, â€Å"The power of population is indefinitely greater than the power in the earth to produce subsistence for man.† Albert Einstein might argue, on the other hand, â€Å"Necessity is the mother of all invention,† albeit in another context. So, which is it? Are we doomed to unchecked population growth followed by Malthusian catastrophe, or can we avoid it through increased food production, decreasing population growth rates, or some other means? To say Malthusian catastrophe is inevitable is completely unwarranted. Is it possible? Certainly – it is only logical that if human population reached levels which far outstripped food supply, the resulting global famine would create easily ignitable tensions between nations, and†¦show more content†¦This spread in the demographic data available to contemporary researchers show that as nations become more developed, fertility rates do not increase exponentially as predicted, nor even linearly, but eventually plateau or potentially even decrease. This concept, demographic transition, has many contributing factors, many of which are uncertain. Still, even if these influences are poorly constrained, the overall trend towards replacement rates of reproduction is well established. The best example of steady and sustainable population levels is the European Union. The EU is highly industrially and agriculturally developed, yet has growth rates near zero percent, and not onl y a sufficient domestic food supply, but an exportable surplus of grains. Some detractors may be quick to point out that this is not the case in many developing countries like India and Sudan, which face shortages of food and resources that are exacerbated by their rapid growth rate. While true, such criticisms do not stand up to the wealth of data showing a strongly correlated positive feedback loop between increasing education and economic gains and falling fertility rates. That is, once such nations in the second phase of demographic transition receive or implement educational and economic improvements, the resulting chain-reaction can move them into the third phase of sustainability before the population can outgrow their resources.

Watching my parents try to climb up the ranks of their job and society, in general, in the most honest way, only to just be met with racist legislation, police brutality, racial profiling, and exploitation, I have come to the conclusion that the American Dream is a fallacy. I’ve seen too many people beat and killed by the system; too many people of color systemically beat down and killed. I have had too many friends fall victim to the prison system. I have worked too many years in the strawberry, blackberry, and blueberry fields. I am tired of seeing my people suffer; it is time for me to help. Naresh Goyal, who already owned JetAir (Private) Limited (which provided sales and marketing for foreign airlines in India) took advantage of this opportunity by setting up Jet Airways as a fullservice scheduled airline that would give competition to state-owned Indian Airlines. Indian Airlines had enjoyed a monopoly in the domestic market between 1953, when all major Indian air transport providers were nationalised under the Air Corporations Act (1953), and January 1994, when the Air Corporations Act was repealed, following which Jet Airways received scheduled airline status. Jet Airways’ 45 destinations include most of the big cities in India. International destinations include Kathmandu, Colombo, Singapore, Kuala Lumpur, Londons Heathrow Airport, Bangkok, Brussels and Newark. Jet Airways was the first private airline in India to fly to international destinations. It started international operations in March 2004 between Chennai and Colombo after it had been cleared by the Government of India to operate scheduled services to international destinations. MACRO ENVIRONMENT FACTORS: Economic Economic environment of India and world affects the airline industry to a great extent. Factors like fluctuations in global fuel prices, exchange rates, slowdown etc have varying impact on the way Jet Airways has been operating in this sector. One of the most important aspect that has greatly affected the worlds transportation system is the high fuel price. Besides high taxes, jet fuel prices have become costlier than crude oil prices which have greatly affected the bottom-line for many airlines including Jet Airways. State taxes on ATF available through government agencies vary between 4% and 30%, making the fuel 30% to 50% costlier than the global average. In Tamil Nadu, Karnataka, Bihar, Madhya Pradesh, Gujarat, West Bengal, Maharashtra and Himachal Pradesh the cess is 25% or higher. The three listed airline companies – Jet Airways, Kingfisher and SpiceJet – all of which reported losses in the quarter ending December 2011, blamed rising ATF cost for their bad financial performance. In February 2012, the Centre gave a nod to direct import of ATF on a case-to-case basis But once airlines start importing ATF, traffic at the countrys congested ports is likely to increase, making matters worse for exporters and importers. Airlines will also have to factor in the cost of transporting ATF from the various ports in the country to airports around the country, something which could eventually lead to increase in the ATF cost. Jet Airways in its financial report has also indicated the losses that it has incurred due to rupee depreciation. It has been reported that revenue from exceptional items decreased by 61% from Rs. 18,919 lakhs in Fiscal 2011 to Rs. 7,319 lakhs in Fiscal 2012 mainly on account of depreciation of the Rupee against the Dollar in the current year. Even in the past Jet Airways has experienced slowdown because of global recession. In 2008 during recession, Jet Airways was forced to discontinue the following routes: Ahmedabad–London, Amritsar–London, Bangalore–Brussels and Mumbai–Shanghai– San Francisco. It also had to put an indefinite delay on its expansion plans. Naresh Goyal, Jet Airways promoter is unable to put in more funds via his holding company due to FDI restrictions. He holds 80% in Jet Airways through Tailwinds. He claims NRI status, where 100% ownership is allowed. However, Goyal’s investment is via Tailwinds, which is considered FDI by the Indian government. Tailwinds is registered in the Isle of Man. In India, only 49% FDI investment is permissible in aviation. Airline business is a complicated one and only airlines would be interested in investing domestic carriers. The government is looking at allowing foreign airlines to invest in domestic carriers but there would be no change in FDI cap of 49%. In this 49%, currently foreign airlines are not allowed to participate. So if this change happens then there would be inflow of funds from foreign airlines. The announcement of ECB (External Commercial Borrowing) with a limit $1 billion in the Budget 2012 will also give some relief to debt ridden airline companies. Air travel is finding a growing proportion of this additional travel, with smaller families making air travel for the entire family more affordable. The Difference Sports Participation and Title IX Have Made Amanda Sprouse The United States of America was built, ideologically speaking, as an escape from a regimented way of life. It was based, in theory, on freedom of expression. However, it was built by white men, for white men. Every major institution was created through a new constitution; a set of laws that would allow men the freedoms they were not capable of attaining in their homeland. Not once were women taken into consideration; at that time it was absurd to even think of allowing women these very same freedoms. Since the beginning, the very socialization of women was against them living their lives as free individuals. Hundreds of years later, much has changed for women. Perhaps one of the most dramatic shifts has taken place in the world of female athletics where, to a certain degree, the establishment of professional athletics for women is now a reality. This is due, in large part, to the introduction of the Educational Amendments of 1972. Title IX, which came from these amendments, states that: No person in the United States shall, on the basis of sex, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any educational program or activity receiving Federal financial assistance. Although athletics are not the specific target for Title IX, they have become a focal point due to the fact that it governs colleges, elementary schools, and high schools. As mentioned above, the patriarchal system in which we live has led to the political socialization of women as inferior to men. Two examples of this are that our constitution refers to a human being as a man and that women are expected to give up their name, an important part of one's identity, when they marry. This argument includes a socialization basis of sexism, which states that the inequality of sexes is based on the variation of the socialization each receives. This way of thinking was evident in every inch of women's lives. It held them back in exercise, but more importantly, it held them back in education and employment opportunities. Vast improvements have been made over the years, particularly in the last twenty-five since Title IX was enacted. In 1972, women earned 9% of medical degrees. Three years ago, that percentage was up to 38%. The percentages look better in terms of law degrees, where women went from earning 7% of law degrees to earning 43% of them. Since 1977, the percent of females earning doctoral degrees moved from 25 to 44%. All of these increases give reason to celebrate what Title IX has accomplished. However, for those who have been in constant battle in the long war against sexism, these statistics are seen as long past due and still not up to par. Sex discrimination has been passed down from generation to generation and even with the women's movement and laws like Title IX, the fight continues to be lop-sided and in f avor of men. There are many complaints about the slow moving progress of Title IX. However slow this progress may seem, statistics indicate improvement and that is worthwhile. This is particularly obvious in the athletic arena: Female participation in athletics has increased dramatically since 1971, when there was only an 8% participation rate among high school girls. By the 1994-95 school year, it rose to a staggering 40%. Meanwhile, boy's participation has remained fairly stable. These statistics are a stumbling block to the arguments of Title IX opponents. They claim that Title IX should not be enforced in college athletics because there is a lack of athletic interest on the part of college females. The fact that high school participation is only 10% away from a 50-50-participation rate indicates a lack of opportunity in college athletics, not a lack of interest. Quite obviously, women today have an unprecedented amount of freedom compared to those raised a mere twenty-five years ago. Today, more girls are encouraged to participate in sports and other activities. They are able to dream about being bigger and better than girls of the past. Studies found that the average prison time for a juvenile sentenced in adult facilities is 3.5 years, which is only about 27% of the imposed sentence. Project Zeros mission is to understand and enhance learning, thinking, and creativity in the arts, as well as humanistic and scientific disciplines, at the individual and institutional levels. The research programs are based on a detailed understanding of human cognitive development and of the process of learning in the arts and other disciplines. They place the learner at the center of the educational process, respecting the different ways in which an individual learns at various stages of life, as well as differences among individuals in the ways they perceive the world and express their ideas. Conceptual Groundwork (1967-1971): During its early years, PZ consisted of a loose collection of 10-15 research assistants and senior scholars. Included in this group were: psychologist Paul Kolers, philosopher Israel Scheffler, literary analyst Barbara Leondar, and Howard Gardner and David Perkins, as founding (and sometimes uncompensated) research assistants. The group met regularly to discuss philosophical, psychological, and conceptual issues in the arts and art education. From the first, the Project took a cognitive view of the arts, viewing artistic activity as involving mental processes fully as powerful and subtle as those used in the sciences or public policy. In that sense, the Project reflected the Cognitive Revolution of the timecountering both the behaviorist past of psychology and the overly romantic view of the arts as matters of mystery, emotion, or entertainment. The Bible for this period was Goodmans influential Languages of Art (1968). During this early period position papers were written, and mode st experiments were undertaken. The results of this first phase of work are captured in a final report for the U.S. Office of Education, prepared by Goodman, Perkins, and Gardner, called Basic Abilities Required for Understanding and Creation in the Arts (1972). During the first years of PZ, Goodman also served as impresario for a dozen memorable lecture-performances at GSE. The purpose of these lecture-performances was to introduce GSE students, and the Harvard community more broadly, to the cognitive processes that characterize artistic planning, performance, and production. In later years, Goodman continued to serve as producer for a series of artistic activities and events at Harvard: these included newly commissioned multimedia performances of John Updikes Rabbit Run (1970), multimedia presentations inspired by Katharine Sturgis drawing series Hockey Seen, and Picassos drawings after Velasquez. Goodman was also catalytic in the formation of the Harvard Summer School Dance Program and the Harvard Business School Program in Arts Management. 2. Empirical Research in Cognitive and Developmental Psychology (1971/2-1983): In 1971, Goodman announced his intention to retire from PZ. He told Gardner and Perkins that they could direct the projectquipping, in characteristic fashion, that means you can raise the money from now on. At first, Perkins took on the directorship and he was joined in 1972 by Gardner. During the following decade, Gardner, Perkins, and a small group of researchers that included Laurie Meringoff (Brown), Ellen Winner, and Dennie Wolf focussed their attention principally on empirical work in the area of cognitive psychology, with a continuing emphasis on artistic issues. An informal division of labor took place, with Gardner and colleagues focussing primarily on developmental issues and populations, while Perkins and colleagues worked primarily with adult artists (and other adult populations). Results of this work can be found in many books and articles, and a number of collections, including Perkins and Leondar The Arts and Cognition (1977) and Gardner and Perkins Art Mind and Educat ion (1989). During this period, much of PZ research was focussed on the arts. Yet across the organization, there was an increased desire to examine issues that went beyond the arts, to look at issues like problem solving, critical thinking, and brain organization. Even in these cases, however, researchers typically were informed by the artistic focus of PZ. 3. The Turn to Education (1983-1993): In 1983, the influential report A Nation at Risk was issued by the U.S. Department of Education. This report catalyzed a lengthy re-examination of American public education as well as newly energized research and development efforts on educational issues at the University Level. PZ had always been housed at the Graduate School of Education and various members had been involved in educational questions and research over the years. Nonetheless, by 1990 PZ had a very different feeling than it had in 1970 (philosophical and conceptual) or in 1980 (primarily psychological research funded by governmental grants). The staff was larger (an average of 50 persons, rather than 20); more of the staff had backgrounds in education rather than in psychology or other scholarly disciplines; and more of the research and development was of a distinctly applied nature. Funding came increasingly from private foundations. At this time, PZ became involved with schools that were based on multiple intellige nces; smart schools that encouraged creative and critical thinking; and collaborations like ATLAS, that included the Coalition of Essential Schools, the School Development Program, and the Education Development Center. In 1992, PZ celebrated its twenty-fifth anniversary at a gala at the Fogg Art Museum. A few hundred individuals attended. Among the speakers were founder Goodman, long term friend Jerome Bruner, and Harvard Deans of Education Theodore Sizer, Patricia Graham, and Jerome Murphy. At the time it was evident that PZ had gone through a number of changes. Compared to the early years, it was much larger; more empirically oriented; extended well beyond the arts; and had a strong applied division, which worked in the schools, museums, and other educational institutions. 4. The Maturing of PZ (1993-2003): PZ has always been self-supporting. In the 1960s and 1970s support came chiefly from the federal government (NSF, NIH). Since the early 1980s support has come increasingly from private foundations. Thanks in significant part to core support from the Atlantic Philanthropic Services Co., Inc. (now The Atlantic Philanthropies), PZ had the opportunity in the last decade to consolidate and mature. We built up a strong central core administration. The former Development Group (led by Gardner) and Cognitive Skills Group (led by Perkins) gave way to a loosely knit but cohesive organization with many internal and external ties and arrangements. PZ became much more prominent on the national and international scene. Details on this most recent period constitute the remaining parts of this report. During the 1990s, Project Zero went through various attempts at reorganization. Each of these phases advanced PZ, and, in retrospect, each seemed a necessary stage of growth. These are used by various criminal justice groups as a measure of crimes solved by the police. Clearance rates can be very problematic for measuring the performance of police services and for comparing various police services. This is because a police force may employ a very different way of measuring clearance rates. For example, each police force may have a different method of recording when a â€Å"crime† has occurred and different criteria for determining when a crime has been â€Å"cleared. One police force may appear to have a much better clearance rate simply because of its calculation methodology. ”Pride and Prejudice” by Jane Austen Essay

Pride and Prejudice is a 19th century novel written by Jane Austen. As it is a popular novel it has been remade for television and film several times. In this essay two media adaptations are being analysed. The 1995 BBC classic drama of pride and Prejudice and a 2004 Bollywood version renamed Bride and Prejudice. The BBC adaptation of Pride and Prejudice is a period drama and has been made using the books storyline and text very closely whereas the Bollywood adaptation has only faithfully adhered to the storyline and parts have been changed to attract a modern audience. In addition to storyline, and text not changed in the BBC adaptation, costume, props and language are all in keeping with the 19th century way of life. However the Bollywood adaptation uses costume, props and language from the Indian culture, this has up dated the film and also keeps with the storyline of the book. Both adaptations have very different audiences they are trying to attract. Pride and Prejudice aims for older and more mature people, an audience who are interested in period dramas and may have read the book. The Bollywood film, however is aimed at younger audiences who may have not heard of Pride and Prejudice nor read the book. It has been changed into a romantic comedy; â€Å"romcom† rather than a more serious look at the book. The use of the Indian culture and view on marriages is a contrast to the views in the 19th century. In both productions the main theme is marriage but they use different methods to represent it. The BBC version shows how relationships and marriage were in the 19th century, in contrast to this how many of the restrictions placed in the 19th century are still in practise in the Indian culture today, however, modernizes the story. In this essay the two media adaptations, (Pride and Prejudice and Bride and Prejudice), are going to be compared to show how they have been adapted for screen. Three key scenes from both productions are going to be discussed and a number of factors to show how they have changes. The three scenes are: * The Netherfield Ball * Mr Collins/Kholi Arrival * Lydia/Lucky and Wickham relationship The Netherfield ball is the place in both adaptations where Darcy and Elizabeth/Lalita meet for the first time the two settings however are very different. In the Bollywood version the setting very much reflects the Indian culture, it is colourful and brash. The atmosphere is hectic with lots of people talking, dancing, laughing and partying. On the other hand the BBC version is more formal with talking kept to minimum and the mood is soft, this is to show what an occasion in 19th century would have been like. The BBC also has kept to tradition and there are different rooms for occasions throughout, Bollywood have used an open plan room to show the change in time and fashion. The camera shots in the scene change a lot. The BBC adaptation uses a birdseye view of the dancing and uses long shot, medium shot, close ups and point of view shot. This is to give a wide variation of the view of the ballroom and closer shots to see the actors’ facial expressions. In the Bollywood adaptation however they use only a few birdseye views of the dancing and more medium shots and point of view shots especially with Darcy and Lalita’s conversation, this shows us what the other person is feeling as they talk to one another and identify with the character. When Darcy is looking up at Lalita on the balcony and Lalita to Darcy on the ground the camera angle is pointed up and down, this gives the film a more dramatic look. Lighting also varies considerably in the two adaptations. In the Bollywood adaptation top lighting is used to brighten the room and give a party atmosphere, this changes in the scene with Darcy and Lalita on the balcony however and backlighting is mainly used. As the BBC adaptation is a period drama lighting is not as bright as the Bollywood effect, there is more under lighting used from lamps and chandeliers. The sound in both adaptations is also clearly different, the BBC use classical sounds and people play instruments for music, and the music they dance to is soft and quite compared to the Bollywood sound. Instruments again produce the music but everyone sings which gives it more volume, they also use words to reflect their feelings. Both scenes have dancing involved but they are clearly different. The BBC has again stuck with traditional dance, which has very repetitive dance moves and is slow. Also shown is how social etiquette is set out with the men only allowed to ask women to dance and how it was very rude for them to refuse. Bollywood has changed this in its adaptation; the dancing is traditional for Bollywood style, fast and very enthusiastic. The men start the dance with the women following later, the women all walk down the balcony and tighten their saris before they dance to show they are ready. Another difference between dances in Bollywood is the man and women dance towards each other rather than with each other, both of these points show how women now have more power than in the 19th century. The songs in Bollywood films offer meaning and feelings of the characters which the film hasn’t got time to express, this is something used in Bollywood and has been included to reflect this. The conversations that take place between Darcy and Lalita and Darcy and Elizabeth are very different. In the BBC version Darcy and Elizabeth do not actually have a face-to-face conversation, he makes a very rude and cutting remark about her which she later makes light of with her friends. The conversation in the BBC adaptation is also very polite and minimal but important, the conversation has not been changed from the book. However, in the Bollywood version Darcy tells Lalita how he thinks it is simpler to have an arranged marriage, she takes this the wrong way and thinks he means Indian women are simple. Lalita is very defensive towards him and tells him otherwise. This change made in the film is to display thoughts on Indian marriage and show the similarities it has with the original 19th century tradition. There is also a conversation which is both in the BBC and Bollywood adaptations of the novel, that of Mrs Bennet/Bakshi. In the scenes she talks about marriage and wealth to f riends, this conversation gives an insight into the character already and has been included in the Bollywood adaptation because the characters are the same. The costumes in both productions are exactly the same for the time. The costumes also represent the characters. Darcy is uncomfortable but more so in the Bollywood version because he cannot speak the language, cannot join in with the dancing and is a different nationality. The Bingley sister wears modern and expansive clothes, in the BBC version she wears feathers and gold jewellery. This has been changed in the Bollywood adaptation to the sister wearing designer labels, this equally shows the wealth but also the change in time. The Bennet/Bakshi sisters in both productions are not as wealthy as the Bingley sisters and this is visible in their clothes. In the BBC adaptation they do not wear feathers and the accessories are very simple likewise in the Bollywood adaptation they do not wear designer labels. In the scene the personalities of the characters are seen and they are quite similar in both adaptations. In the Bollywood adaptation Darcy is uncomfortable, however he also very rude in the BBC version. Elizabeth is lively, strong and defensive in both scenes, she also shows in the BBC adaptation when she makes light of Darcy’s comment about her. Bingley in both versions is charming and in Bollywood takes part in the dance unlike Darcy. Mrs Bennet and Bakshi both talk about marriage and don’t differ at all in the adaptations, as do the younger Bennet sisters they are flirtatious and playful. The key personality of the characters has not been changed at all and this is one way in which the Bollywood adaptation has stuck to the novel. The scene of Mr Collins/Kholi’s arrival in both adoptions is changed in many ways. Transport that Mr Collins/Kholi arrives in is very different, in the Bollywood adaptation Mr Kholi arrives in a taxi whereas in the BBC adaptation Mr Collins arrives in a carriage. This change shows the change in times and the change of country. The setting is also different and reflects the change in times and country, Mr Collins arrives out side the Bennets house in the Bennets estate which has gardens and gravel walkways, Mr Kholi however, arrives on a dusty Indian street and the Bakshis do not own any estate around them. This to show how the Bakshis live compared to the Bennets. In terms of camera shots Mr Collins/Kholi’s arrival is quite the same, the camera use medium and long shots to show both characters arriving and point of view shots from the families and Mr Collins/Bakshi’s view. Both adaptations also use close up shots of the sisters smirking at Mr Collins/Kholi, this involves the audience in the amusement they find in Mr Collins/Kholi. Lighting is different in the scenes though, Mr Collins arrives in daylight at the Bennets and toplighting is used to give a pleasant atmosphere. In the Bollywood adaptation they are inside and the lighting is darker but again toplighting is used. There are no songs or music in the scenes of Mr Collins/Kholi arriving there is only he and the families talking, however in both films sound effects are used. In the BBC adaptation the carriage arriving and in the Bollywood adaptation the Indian street, these sound effects add to the atmosphere and setting of the arrival. Another adaptation that has been made is the costume. Mr Collins wears formal dress which is the clothes which would be worn in the 19th century. The style of his clothes are also a representation of him, he is a clergyman and a very plain man. Mr Kholi’s clothes are stylish, bold and very up to date, and this represents his character very well. Mr Kholi is self-confident, loud man and does not realise often what he is saying or how he offends people, the clothes also represent his break away from the Indian culture and living in America. As well as the costumes reflecting their characters the audience can also see what the characters are like. In the BBC adaptation Mr Collins is a religious and traditional man, Mr Kholi however has turned his back on his Indian culture and moved to America for a better life. There are some parts of the Mr Collins character that are the same in the Bollywood adaptation in the same way as the BBC adaptation. Both Mr Collins and Mr Kholi look do wn to the Bennets/Bakshis, they think they are better than them. In Mr Collins’ case this is because he will inherit the Bennets estate and Mr Kholi’s because he has more money and a better life than the Bakshi’s, they are both unpleasant characters. The Bollywood adaptation has also changed the conversation of Mr Kholi compared to Mr Collins. When Mr Collins arrives he uses his language to make himself look smart, he uses substantial and intelligent words, which are incorrect. Alternatively Mr Kholi brags about his wealth and lifestyle, he also has inappropriate conversations and is very out spoken. Greetings in the two adaptations are also different too, Mr Collins greets people formally and there is no touching or hugging, this reflects the social etiquette of the 19th century instead of the informal hugs Mr Kholi gives. In both adaptations there are similarities and differences in the mannerisms of Mr Collins and Mr Kholi. Mr Collins uses formal language and, as mentioned before, incorrect in the context intelligent words. This is unlike Mr Kholi who uses casual language and does not realise what he says sometimes offends people. In the same way Mr Collins eats using social etiquette and Mr Kholi eats with his hands and mouth open, Mr Kholi also uses his hands when he talks. These changes show equally how the character is offensive but in different ways and in different circumstances. A similarity however, is that Mr Collins/Kholi feel they are more important than certain people and this shows when they both sit at the head of the table at the meal with the Bennets/Bakshis. As well as the mannerisms of Mr Collins/Kholi presenting similarities and differences, people’s reactions to Mr Collins/Kholi do too. Both of the eldest sisters in the Bennets/Bakshis smirk at each other when he says things and tries to boast about his life, this shows they are sensitive and intelligent enough to realise how stupid Mr Collins/Kholi is. Mr Bennet/Bakshi realises this too but expresses his amusement in different ways. Mr Bennet makes sarcastic comments towards Mr Collins to show him up but Mr Collins does not understand, Mr Bakshi however uses facial expressions to make fun of him. This reflects the characters of Mr Bennet and Mr Bakshi, Mr Bennet takes a more serious approach where Mr Bakshi uses childish gestures. Furthermore Bollywood has also made a change with the reaction of Darcy when he meets Mr Kholi. In the BBC adaptation Mr Collins introduces himself to Darcy and talks about his self, Darcy walks away and ignores him in contrast, Darcy is introduced t o Mr Kholi and does not ignore him. As shown by Mr Bennet and Mr Bakshi this is a reflection of Darcy’s characters, Darcy in the BBC adaptation is confident enough to be able to walk away but Darcy in the Bollywood adaptation is not, as he is a country he does not know he is polite to everyone. The reaction of people towards Mr Collins/Kholi reveals a lot about his character; he is someone who tries to be popular by knowing everyone and likes to be well thought of. One relationship, which has been adapted and changed considerably, is the Lydia/Lucky relationship. Adapted for Bollywood are a number of scenes where the main action takes place. In the BBC adaptation the scenes take place after Wickham and Lydia have eloped and are living in a grotty bedsit, this has changed for Bollywood and is replaced by scenes in public places e.g. fairground, cinema and the main scene where Lucky and Wickham go out for the day in London. The scene has been changed for Bollywood because Lucky and Wickham are not actually together yet and it is the lead up for Lucky finding out eventually who Wickham really is after pursuit by Darcy and Lalita. This change has been made for the viewer to look at their relationship in two different ways, in the BBC adaptation Lydia and Wickham have to stay in bedsit which is far form luxury, this shows how their relationship is very shameful in the 19th century. In contrast to this Lucky and Wickham meet at very public places, th eir relationship seems very open and would not be seen wrong in the public nor the viewer’s eye, but the viewer understands Wickham is a bad man. The use of media effects in both adaptations is strong. The camera effects in both adaptations are similar, in the BBC adaptation medium shots and close ups are used to give the viewer a more intimate view of Lydia and Wickham. It shows shots of Lydia running around the bedsit and close ups of Wickham’s facial expressions to the viewer to tell them he is not happy about the situation. In the Bollywood adaptation the camera plays a very important part in the chase that occurs between Wickham, Lucky, Lalita and Darcy. The camera use close ups and medium shots of the couples, and glances between the two parties. It also uses close ups of Wickham glancing back to see if they are still being chased. The camera follows Lucky and Wickham but blurs out the background to give the viewer an impression of how easily it would be to lose them in a crowd. The lighting effect in the BBC adaptation gives the atmosphere of a dark room and uses underlighting, the only available light source the viewer can see is the daylight coming through the window. Two different types of lighting is used in the Bollywood adaptation however, the first toplighting used in the chase to create a bright, modern setting for the viewer. This contrast however when Lucky and Wickham go into the cinema, the lighting is backlighting from the cinema screen and also presents to the audience that Wickham is trying to hide. Costume has also been changed considerably, the BBC adaptation use typical dress for the 19th century but change it from formal wear to very inappropriate wear in the bedsit. Lydia wears a nightdress and Wickham stays in his uniform but un-tucked his shirt and his boots are undone, this is to represent the seediness of their relationship and how wrong it was in the 19th century. Lucky and Wickham are the same in that Lucky tries to rebel against her Indian culture, like the rules set out in the 19th century, in the scene where Lucky and Lydia meet in London she wears tight jeans and a cropped top and makes no effort to show her Indian roots. Wickham also wears clothes that are very casual and he looks like a traveller, this is in contrast to Darcy’s clothes which are smart, and reveals that Wickham may not be the good character as seen by Lalita and Lucky. The costumes in both adaptations have been translated to represent the character in the same way but to show the change in different cultures and times. Lydia and Lucky wear clothes seen as normal and traditional but try to show their flirty ways by tighter clothes but in other scenes they rebel completely. In Lydia’s case she wears a very inappropriate nightdress and Lucky wears modern, tight clothes. It is the same for Wickham too; Wickham is always put in as the opposite of Darcy who is very formal unlike Wickham who wears an officer uniform or traveller’s clothes, this is also to show Wickham is not like Darcy and a bad character. Bollywood have adapted Wickham to become a traveller to be a modern representation of the BBC Wickham who becomes an officer because he has no money. In the scenes of Lydia/Lucky and Wickham the audience can see a lot about their character. Lydia and Lucky both are flirtatious, young girls who are unaware of consequences of their actions; this is displayed through Lydia’s obsession of the officers and her elopement and Lydia through her constant attention seeking of Wickham. Wickham also plays the handsome man well in both adaptations, his rough look in contrast to Darcy should also tell the viewer he is not as he seems. There are many things left out of the Bollywood adaptation that are in the BBC one. In the Bollywood adaptation we see Lucky and Wickham develop a relationship and meet up on occasions, this does not happen with the BBC as Lydia and Wickham, by chance, meet in Brighton and make a quick decision. This is to show the change in times and controversy of what Lydia and Wickham did, also it is not believable that Lucky and Wickham would have gone to London on just one meeting. The chase of lucky and Wickham also adds to the tension and thrill for the audience, this makes the viewer think, will they be caught? There is also one major difference between the BBC and Bollywood adaptation, the ending. In the BBC adaptation Darcy pays Wickham to Marry Lydia and therefore not bringing shame on the Bennets, in the Bollywood adaptation Wickham and Lydia are caught by Darcy and Lalita and Wickham is seen for the real man he is. This has been changed for the Bollywood adaptation for the happy ending that happens in Bollywood films, it would not be entirely like a Bollywood film if Lucky ended up with the bad man and not be happy. In conclusion the BBC and Bollywood adaptations of Pride and Prejudice have been made very differently. The title of the Bollywood adaptation has been changed to Bride and Prejudice to reflect the difference in the Indian culture, the main personality of the characters have stayed the same in both adaptations, costumes have been changed to reflect the difference in cultures and times yet kept so the characters traits can be seen, the contrast in settings shows the different countries and a different way in life and the change in the storyline reflects the difference in times, countries and culture. The setting of the story has been changed as the film wants to show the restrictions on 19th century marriage still coincide with the Indian view on marriages today, the characters traits were kept the same to show how they could be brought into the present day but changed slightly in their costumes, mannerisms and they way people see them. As the characters are represented through their costumes, the wealth of the families is too, the costumes have been changed in the Bollywood adaptation to traditional Indian costumes but they show the contrast to the expensive clothes of the Balraj sister compared to the Bakshi sisters. This is also the same for the Bingley sister and Bennet sisters in the BBC adaptation. I preferred the Bollywood adaptation. This is because it is not as serious as the BBC adaptation; it is more vibrant, colourful and loud. The singing and dancing breaks up the film and the comedy also puts more life into the story. The BBC adaptation is too long and does not keep my attention as much as the other. Another reason why I preferred the Bollywood adaptation was the costumes and new surroundings, the Indian culture is very bold and this made it more enjoyable to watch.

Binge eating disorder Essay Binge eating disorder (BED) is characterized by recurrent episodes of binge eating not accompanied by inappropriate compensatory behaviors. Although it is not yet officially recognized, it was included in the current Diagnostic and Statistical Manual of Mental Disorders (DSM) as a disorder to be studied, with research criteria included. It is also slated to be included in the next DSM as one of the officially recognized eating disorders. There have been several treatment modalities employed in its treatment: pharmacotherapy, psychotherapy, and a combination of the two. Being a â€Å"young† disorder, BED still needs future research to show which modality treats it best in the long run. Approaches to Treatment of Binge Eating Disorder What Is BED? How Is It Different From Bulimia Nervosa? Binge eating disorder (BED) is characterized by recurrent episodes of binge eating not accompanied by inappropriate compensatory behaviors (Sadock Sadock, 2003). Although binge eating, by itself, is more commonly associated with bulimia nervosa, binge eating disorder is recognized as a separate entity. Unlike BED, bulimia nervosa involves excessive concern with body shape and weight, and bulimics often have a â€Å"self-perception of being too fat, with an intrusive dread of fatness† (World Health Organization [WHO], 1993). Moreover, the absence of compensatory behaviors further distinguishes BED from the non-purging type of bulimia nervosa, which is defined by the Diagnostic and Statistical Manual of Mental Disorders, or DSM-IV, as when the individual â€Å"has used other inappropriate compensatory behaviors, such as fasting or excessive exercise, but has not regularly engaged in self-induced vomiting or the misuse of laxatives, diuretics, or enemas† (American Psychiatric Association [APA], 2000). Diagnosis of Binge Eating Disorder Although binge eating disorder still has to gain official recognition, the Text Revision of DSM-IV (APA, 2000) lays down the following â€Å"research criteria† for diagnosing the disorder: A. Recurrent episodes of binge eating. An episode of binge eating is characterized by both of the following: (1) eating, in a discrete period of time (e. g. , within any 2-hour period), an amount of food that is definitely larger than most people would eat in a similar period of time under similar circumstances (2) the sense of lack of control over eating during the episode (e. g. , a feeling that one cannot stop eating or control what or how much one is eating) B. Binge-eating episodes are associated with three (or more) of the following: (1) eating much more rapidly than normal (2) eating until feeling uncomfortably full (3) eating large amounts of food when not feeling physically hungry (4) eating alone because of being embarrassed by how much one is eating (5) feeling disgusted with oneself, depressed, or very guilty after overeating C. Marked distress regarding binge eating is present. D. The binge eating occurs, on average, at least 2 days a week for 6 months. E. The binge eating is not associated with the regular use of inappropriate compensatory behaviors (e. g. , purging, fasting, excessive exercise, etc. ) and does not occur exclusively during the course of anorexia nervosa or bulimia nervosa. Several issues have been raised regarding the criteria mentioned above. For example, one question raised by some experts in the field is that there is no definite way to assess just how much food constitutes â€Å"an amount of food that is definitely larger than what most people would eat in a similar period of time under similar circumstances. † Fulfilling these criteria would at best be a subjective process. While the DSM-IV criteria for BED is usually applied to adults, Marcus and Kalarchian (2003) have proposed a separate set of provisional research criteria for diagnosing BED in children. These are: A. Recurrent episodes of being eating. An episode of binge eating is characterized by both of the following: (1) food seeking in absence of hunger (e. g. after a full meal) (2) a sense of lack of control over eating (e. g. , endorse that ‘‘When I start to eat, I just can’t stop’’) B. Binge episodes are associated with one or more of the following: (1) food seeking in response to negative affect (e. g. , sadness, boredom, restlessness) (2) food seeking as a reward (3) sneaking or hiding food C. Symptoms persist over a period of 3 months. D. Eating is not associated with the regular use of inappropriate compensatory behaviors (e. g. , purging, fasting, excessive exercise) and does not occur exclusively during the course of anorexia nervosa or bulimia nervosa. Some of the questionnaires used to diagnose binge eating disorder are: the Binge Eating Scale (BES), the Three Factor Eating Questionnaire, the Body Shape Questionnaire, the Structured Clinical Interview for the Diagnosis of DSM Disorders (SCID), and the Eating Disorders Examination (EDE). The Treatment of Binge Eating Disorder Overview In the management of BED, the primary goal is to achieve abstinence from binge eating (Bulik, Brownley Shapiro, 2007). However, studies have shown that BED has comorbid conditions that require as much attention, such as depression, generalized anxiety disorder, panic attacks and even attempts at suicide (Grucza, Przybeck Cloninger, 2007). Furthermore, since patients diagnosed with BED do not usually engage in inappropriate compensatory behaviors, they are usually obese as well, a condition that sometimes causes more concern in professionals caring for BED patients because of its health implications. Because of the different aspects of this disorder, different treatment modalities are being used, with some experts favoring one and others favoring another. Pharmacotherapy Medications used to treat binge eating disorder include appetite suppressants, anticonvulsants, selective serotonin and norepinephrine reuptake inhibitors, and tricyclic antidepressants. These have been studied in double-blind placebo-controlled trials, and the results, though modest, have been promising (Appolinario McElroy, 2004). Appetite suppressants. According to Appolinario, Bacaltchuk, Sichieri, Claudino, Godoy-Matos, Morgan, Zanella Coutinho (2003), fenfluramine hydrochloride was the first antiobesity agent shown to be effective in treating binge eating disorder. However, this drug has been withdrawn from the market because of intolerable side effects. Currently, the anti-obesity agent that seems to be the most studied in the treatment of BED is sibutramine. Sibutramine is a drug that inhibits the reuptake of serotonin, norepinephrine, and, to some extent, dopamine. A study by Appolinario et al. (2003) showed that patients taking sibutramine, as opposed to those given a placebo pill, experienced a significant reduction in the number of days with binge episodes. Whereas those in the placebo group actually gained some weight, the patients given sibutramine lost an average of 7. 4 kg. The study participants were asked to fill out the Binge Eating Scale and the Beck Depression Inventory before and after their treatment program, and the sibutramine group had significantly lower scores after treatment. The study cites dry mouth and constipation as the adverse reactions commonly encountered by those taking sibutramine. The authors concluded that sibutramine is effective and well tolerated for treating obese BED patients. Another study supporting the use of sibutramine in BED was published in the American Journal of Psychiatry in January 2008. Wilfley, Crow, Hudson, Mitchell, Berkowitz, Blakesley, Walsh, and the Sibutramine Binge Eating Disorder Research Group (2008) found that patients taking sibutramine (15 mg daily for 24 weeks) were able to significantly reduce the following measures: weekly binge frequency, weight (mean of 4. 3 kg), frequency of binge days, body mass index, and eating pathology, such as cognitive restraint, disinhibition and hunger. Moreover, these patients also experienced global improvement and had a greater percentage of abstinence from binge eating (58. 7%, compared with the 42. 8% of the placebo group). Anticonvulsants. In the February 2003 edition of the American Journal of Psychiatry, McElroy, Arnold, Shapira, Keck, Rosenthal, Karim, Kamin Hudson (2003) reported that 30 patients with binge eating disorder randomly assigned to receive a flexible dose (25 to 600 mg per day) of the anticonvulsant drug topiramate had a significantly greater rate of reduction in binge frequency, binge day frequency, weight (mean loss of 5. 9 kg), and body mass index. They also had lower scores on the Yale-Brown Obsessive Compulsive Scale, which was modified for binge eating, and on the Clinical Global Impression severity scale. However, six patients receiving topiramate were unable to continue participating in the study because of adverse events, mostly headaches and paresthesias. The researchers concluded that topiramate was useful for the short-term treatment of BED. The journal Biological Psychiatry also published a study by McElroy, Hudson, Capece, Beyers, Fisher, Rosenthal, and the Topiramate Binge Eating Disorder Research Group (2007), which showed that 195 patients taking topiramate were able to reduce binge eating days per week, binge episodes per week, weight (mean loss of 4. 5 kg), and body mass index compared with those assigned to take placebo. Thirty percent of those taking topiramate left the study, mostly due to adverse effects such as paresthesia, upper respiratory tract infection, somnolence and nausea. However, thirty percent of those taking the placebo pill also were not able to continue with the program. Despite the adverse events reported, the authors concluded that topiramate is well tolerated and was efficacious in treating BED and its features, particularly obesity. Another antiepileptic drug that was evaluated for efficacy in the treatment of binge eating disorder is zonisamide. McElroy, Kotwal, Guerdjikova, Welge, Nelson, Lake, D’Alessio, Keck Hudson (2006) reported in the Journal of Clinical Psychiatry that thirty patients given zonisamide (100 to 600 mg per day for 16 weeks) had a reduction in the frequency of their binge eating episodes, body weight, body mass index, and scores on the Yale-Brown Obsessive Compulsive Scale Modified for Binge Eating and the Clinical Global Impressions Severity Scale. Eight of the patients receiving zonisamide discontinued treatment because of accidental injury with bone fracture, psychological complaints and cognitive complaints. In conclusion, the researchers noted that zonisamide was efficacious in treatment of BED with obesity but that it was not well tolerated. Selective serotonin reuptake inhibitors (SSRIs) and norepinephrine reuptake inhibitors. Several selective serotonin reuptake inhibitors have been shown to have beneficial effects in the management of binge eating disorder. Among the earliest SSRI’s shown to be useful in BED treatment was fluvoxamine. Hudson, McElroy, Raymond, Crow, Keck, Carter, Mitchell, Strakowski, Pope, Coleman Jonas (1998) reported that forty-two patients who received a flexible dose of fluvoxamine (50 to 300 mg) for 9 weeks had a significantly greater decrease in the frequency of binges and in body mass index. Furthermore, these patients given fluvoxamine achieved a greater reduction in Clinical Global Impression severity score and a greater rate of increase in Clinical Global Impression improvement scores. However, fluvoxamine did not have a significant effect on the participants’ Hamilton depression scale scores, and the proportion of patients who dropped out of the study because of adverse effects came from the fluvoxamine group. Furthermore, another study done 5 years later on a different, smaller set of patients showed that there was a reduction in binge frequency, eating concern, shape concern and weight concern in both the group receiving fluvoxamine and the group receiving placebo (Pearlstein, Spurell, Hohlstein, Gurney, Read, Fuchs Keller, 2003). The authors say this is just an example of the â€Å"inconsistent results of antidepressant studies in binge eating disorder† (Pearlstein et al. , 2003). Another SSRI shows greater promise. McElroy, Hudson, Malhotra, Welge, Nelson Keck (2003) conducted a 6-week, double-blind, flexible-dose (20 to 60 mg per day) study on the efficacy of citalopram in reducing the frequency of binge eating episodes. They reported that the 19 patients receiving citalopram, compared with those receiving placebo, had a significantly greater rate of reduction in the frequency of binge eating episodes, frequency of binge days, weight, body mass index and severity of illness. In addition, citalopram was said to be well tolerated by the participants of the study. A head-to-head comparison of two SSRI’s – sertraline and fluoxetine – was conducted by Leombruni, Piero, Lavagnino, Brustolin, Campisi Fassino (2008) and published in the journal Progress in Neuro-psychopharmacology and Biological Psychiatry. Twenty-two participants were given 100 to 200 mg of sertraline daily, while twenty were given 40 to 80 mg of fluoxetine per day. After 8 weeks, participants from both groups had significant weight loss and their scores in the Binge Eating Scale improved. There was no significant difference found between the two drugs, and the researchers concluded that both SSRI’s are effective options in treating patients with BED. A drug that inhibits both serotonin and norepinephrine reuptake was studied by Noma, Uwatoko, Yamamoto Hayashi (2008). Although the trial did not focus exclusively on individuals with binge eating disorder but consisted of 25 binge eaters, some of whom were diagnosed with anorexia or bulimia nervosa. These patients received the drug milnacipran for 8 weeks and were found to have improved scores in relation to their drive for binge eating and regret for the same. Milnacipran was noted to be more effective in patients without purging. Venlafaxine, another medication that inhibits the reuptake of both serotonin and norepinephrine, was mentioned by Appolinario et al. (2003) as effective in BED therapy. McElroy, Guerdjikova, Kotwal, Welge, Nelson, Lake, Keck Hudson (2007) examined the effects of atomoxetine, a norepinephrine reuptake inhibitor, on BED. Twenty patients who received the drug had a significant decrease in binge-eating episode frequency, binge day frequency, weight, and body mass index. Their scores on the Clinical Global Impressions Severity of Illness scale, the Yale-Brown Obsessive Compulsive Scale Modified for Binge Eating obsession subscale, and the Three Factor Eating Questionnaire hunger subscale improved as well. However, three of the patients receiving atomoxetine asked to discontinue their treatment because of increased depressive symptoms, constipation or nervousness. Tricyclic antidepressants. Bulik, Brownley Shapiro (2007), in a review of the diagnosis and management of binge eating disorder, also mentions imipramine as one of the drugs that were shown in trials to be effective in the treatment of BED. Imipramine is a tricyclic antidepressant that is used mainly for the treatment of depression and enuresis. A drug from the same family, desipramine, was also mentioned as having beneficial effects in BED. There have indeed been many drugs explored by research for the treatment of binge eating disorder. However, while a lot of studies show that they do have a beneficial effect on patients with BED, particularly with regards to a reduction in binge frequency and weight, the results are somewhat modest. In addition, there have been no long-term studies tracking the efficacy of these drugs as yet. And then, of course, there is the concern that many of those individuals taking these drugs might give them up due to the adverse effects they cause. More importantly, the drugs do not address the underlying cause of binge eating, the psychopathology in individuals that lead them to take in large amounts of food despite the knowledge that doing so could harm them physically and socially. Thus, while many experts in the field would agree that medications help to curb the comorbidities of BED, they still encourage the use of psychotherapy. Psychotherapy Cognitive behavioral therapy. The behavioral therapeutic approach to binge eating disorder that is probably the most tested is cognitive behavioral therapy, or CBT (Bulik, Brownley Shapiro, 2007). As the name implies, the theory behind this practice is the cognitive behavioral model of binge eating, which â€Å"postulates that binge eating develops in response to restrictive food intake and occurs in the context of ongoing dietary restraint and the experience of negative emotions† (Levine Marcus, 2003). In short, inaccurate thoughts and beliefs – for example, those about body shape and weight – lead to inappropriate eating behavior. CBT helps patients identify these binge-triggering thoughts and modify them so as to accomplish binge abstinence. According to Bulik, Brownley Shapiro (2007), cognitive behavioral therapy has been reported by several studies to be effective in reducing â€Å"binge frequency, related psychological aspects of binge eating (restraint, disinhibition, and hunger), depressed mood, and ratings of illness severity in individuals with BED. † Levine Marcus (2003) note that, while cognitive behavioral therapy has been traditionally used in the management of individuals with bulimia nervosa, the technique has been modified to accommodate the differences between bulimia nervosa and binge eating disorder. For example, unlike people with bulimia nervosa, those with binge eating disorder tend to be obese, so CBT can directly target cognitions about having a large body size. They explain, â€Å"Overweight individuals with BED may be helped to accept their body size and to restructure maladaptive thoughts about the amount of weight loss they are likely to achieve. That is, although modest weight loss may relate to improvements in binge eating, for most BED patients this decrease may not correspond with their desired weight loss. It is therefore important that cognitions about acceptable body sizes be targeted during treatment. † Interpersonal psychotherapy. Another type of therapy that has found success in patients with bulimia nervosa and has since been also applied to people with binge eating disorder is interpersonal psychotherapy, or IPT. While CBT focuses on the thoughts that trigger eating binges, interpersonal psychotherapy operates on the theory that binge eating stems from the internal milieu created by specific social and interpersonal problems. Thus, it â€Å"focuses on identifying and addressing specific, problematic interpersonal patterns, in an effort to ameliorate dysfunctional eating behaviors† (Levine Marcus, 2003). As with CBT, IPT is focused, structured and time-limited. However, it does not address the patient’s beliefs about eating, weight and shape and, unlike CBT, does not directly target eating behaviors. In the Archives of General Psychiatry, Wilfley, Welch, Stein, Spurrell, Cohen, Saelens, Dounchis, Frank, Wiseman Matt (2002) report on a comparative study that they conducted between CBT and IPT. Both were done in a group setting, and the participants in each group went through 20 weekly sessions of therapy. Results showed that â€Å"binge-eating recovery rates† were similar for both CBT and IPT immediately after treatment. They further conducted follow-up on each participant and noted that, while, there was a slight increase in binge eating when the treatment ended, the frequency of such binges remained significantly lower than the frequency prior to initiation of either CBT or IPT. In addition, the researchers reported significant reductions in psychiatric symptoms, and these were maintained through follow-up. They noted that, while dietary restraint decreased more quickly in patients who underwent CBT, a similar level of dietary restraint was accomplished by those who underwent IPT by the time of follow-up. The participants’ weight decreased only slightly, but significantly, and the authors then concluded that group IPT is a reasonable alternative to group CBT for dealing with overweight BED patients. Dialectical behavior therapy. Another type of therapy used for the treatment of BED is dialectical behavior therapy, or DBT. This is described by Levine Marcus (2003) as â€Å"a comprehensive treatment program based on cognitive and behavioral principles and complemented by the use of acceptance-based strategies derived primarily from Zen Buddhism. † Patients undergo a weekly individual outpatient therapy and, at the same time, a weekly group skills therapy that has the goal of increasing behavioral skills that can help them deal with binge eating. According to Bulik, Brownley Shapiro (2007), dialectical behavior therapy â€Å"fosters the development of skills in the domains of mindfulness, emotion regulation, interpersonal effectiveness, and distress tolerance. † Although it is still considered an â€Å"alternative† form of behavior therapy, there have been studies showing that DBT has led to a greater reduction in binge days, binge episodes, and concerns with weight, shape and eating. Other behavioral therapies. Aside from cognitive behavior therapy, interpersonal psychotherapy and dialectical behavior therapy, Bulik, Brownley Shapiro (2007) mention self-help, exercise and virtual reality therapy as treatment modalities that have been tried in patients with BED. Self-help interventions are delivered in various formats, and may be done with or without structure, and with out without the aid of a facilitator or a therapist. It has been shown by some studies to result in greater reductions in the mean number of binge days and in the clinical severity of BED. Abstinence and cessation rates were also reported to be improved with self-help, but weight loss was not significantly achieved. Medications and Psychotherapy: Combined and Compared Several studies have explored the option of combining pharmacotherapy with psychotherapy. Bulik, Brownley Shapiro (2007) discuss the results of these studies. For example, a trial compared fluoxetine alone with CBT alone and fluoxetine plus CBT. The authors of that trial reported that CBT plus fluoxetine, and CBT alone, were more effective than fluoxetine alone in reducing the frequency of binges, concerns with eating and body shape, disinhibition and depression. Another comparison discussed in the same paper compared desipramine alone, weight loss therapy, and CBT. Bulik, Brownley Shapiro (2007) relate: â€Å"Binge eating was significantly reduced after 12 weeks in both groups receiving CBT; however, this effect did not persist at 36 weeks of treatment. Average weight loss was greatest in the weight loss therapy group in the early stages of treatment, but over time (i. e. , at 3-month follow-up) the group receiving desipramine lost the most weight. Desipramine showed no clear advantage in reducing symptoms of depression. † Claudino, de Oliveira, Appolinario, Cordas, Duchesne, Sichieri Bacaltchuk (2007) compared topiramate alone with CBT plus topiramate and concluded that â€Å"topiramate added to CBT improved the efficacy of the later, increasing binge remission and weight loss in the short run. Topiramate was well tolerated, as shown by few adverse events during treatment. † Finally, Molinari, Baruffi, Croci, Marchi Petroni (2005) conducted a comparison of CBT alone, fluoxetine alone, and CBT plus fluoxetine. Results showed that â€Å"the two groups which underwent psychotherapy resulted in a better outcome in terms of number of bingeing episodes, maintenance of weight loss reduction from baseline and psychological well being than the group treated with pharmacological therapy alone. † The authors concluded that the results of their study highlight the importance of a multidisciplinary approach to binge eating disorder. However, as with pharmacotherapy alone and with psychotherapy alone, the long-term efficacy of a combined drugs-and-talk approach to binge eating disorder still has to be proven by future studies. Practical Advice To Tell Patients Empowering patients to take control of their disorder is also important. Smith, Barston, Segal Segal (2008) offer the following tips that concerned eating disorder professionals can give their patients: †¢ Eat breakfast. Skipping breakfast often leads to overeating later in the day, so start your day right with a healthy meal. Eating breakfast also jump starts your metabolism in the morning. Studies show that people who eat breakfast are thinner than those who don’t. †¢ Avoid temptation. You’re much more likely to overeat if you have junk food, desserts, and unhealthy snacks in the house. Remove the temptation by clearing your fridge and cupboards of your favorite binge foods. †¢ Stop dieting. The deprivation and hunger of strict dieting can trigger food cravings and the urge to overeat. Instead of dieting, focus on eating in moderation. Find nutritious foods that you enjoy and avoid labeling foods as â€Å"good† or â€Å"bad. † †¢ Exercise. Not only will exercise help you lost weight in a healthy way, but it also lifts depression, improves overall health, and reduces stress. The natural mood-boosting effects of exercise can help put a stop to emotional eating. †¢ Destress. Learn how to cope with stress in healthy ways that don’t involve food. Conclusion Binge eating disorder is still relatively â€Å"young. † With its anticipated inclusion in the next edition of the DSM, a lot of studies have turned their attention to this eating disorder that has only been recognized as an entity separate from the other eating disorder in the last decade. Hopefully, these studies, and the ones still to be conducted, will provide a sound basis for how best to approach binge eating disorder. More importantly, only future research can tell which treatment modalities really work best in the long term. In the meantime, patients with binge eating disorder need help in the here and now.
